Bid Opportunity for Public Health Pharmaceutical Cache Public Health Scope and Classification 1.1 Scope:
Franklin County Public Health (FCPH), along with Columbus Public Health (CPH), and in conjunction with Franklin County Homeland Security and Justice Programs (FCHS&JP), is seeking bids to establish a contract for the immediate purchase of pharmaceuticals for a public health cache.
Contract:
The funding source for this purchase comes from a Columbus Urban Area Security Initiative (UASI) Grant. FCPH will handle the bidding process. FCPH will select the vendor, place the order and approve the delivery, invoice and all work related to this project. The purchase order will be processed and the invoice wil be paid by the FCHS&JP office as they are the holder of the UASI grant funding. The awarded supplier will be required to sign a contract with FCPH and FCHS&JP. The winning bidder must provide a completed W-9. Vendor must submit a certification of compliance regarding ORC 3517.13.
Classification:
Pharmaceuticals to be bid are: Doxycycline and Ciprofloxacin. All bidders must be licensed to distribute pharmaceuticals in the State of Ohio.
Applicable Publications and Standards All manufacturers, product names, and model numbers referred to in these specifications are to establish minimum quality standards for that particular item. Suppliers are invited to bid products of any manufacturer that meets these minimum standards. Bid Requirements 3.1 Term:
The proposed contract shall be in effect from the date of execution through product delivery and inspection.
Pricing:
Bidders are requested to bid firm or fixed prices.
Quality Standard: The supplier shall guarantee the materials and workmanship which enter in to the
manufacture of the items relating to this bid to be the best of their respective kinds and shall guarantee to replace, any part, parts, or entirety of the item(s) which prove defective at no expense.
Expiration Date: Shelf Life: 24-36 months. Lot Numbers: Lot numbers are to remain consistent with the order of each product. Items to Bid:
Total of 1,000,000 doses of 100mg doxycycline, received from supplier to contractor in bulk bottles of 500 dose tablet form. Product must then be repackaged and relabeled to meet the following specifications: 50,000 bottles of 20-count doses per bottle. a.
Shelf Life: Must be 24-36 months when received from the supplier.
A single lot number is to remain consistent with entire order of doxycycline.
Bottles must be packaged in cases and shipped on pallets not to exceed four feet (wide) by four feet (long) by six feet (tall).
Any damaged, defective, or substandard unit shall be removed by the contractor and replaced at no cost.
Total of 1,200,000 doses of 100mg doxycycline, received from the supplier to contractor in bulk bottles of 500 dose tablet form. Product must then be repackaged and relabeled to meet the following specifications: 60,000 bottles of 20-count doses per bottle. a.

FCPH reserves the right to request samples of any item(s) bid. Samples will be requested only when required, and in sufficient quantities to determine the quality of the item bid. 4.2 Damage:

Procedures:
FCPH reserves the right to take selected products to a credited laboratory to assure compliance to specifications.

5.2 Minimum
The bidder shall state on proposal, any minimum quantity required for delivery. If no quantity is specifically stated, it will be presumed that no minimum quantity is required for delivery.
5.3 Containers:
Pharmaceuticals are to be shipped/delivered in containers compliant with manufacturer’s specifications.

Orders will be accepted during regular business hours: Monday-Friday 8:00AM-4:00PM local time, exempting all observed City of Columbus holidays. Notice of delivery date and estimated time is requested. 5.5 Shipping Requirements:
Entire order must be shipped on pallets no larger than four feet (wide) by four feet (long) by six feet (tall).

This proposal is the bidder's offer to sell the item(s) set forth on the bid proposal sheet at the prices(s) quoted by bidder thereon, under the terms and conditions of these bid documents. An estimated quantity is set forth on the bid quotation sheet.
